My feet are much happier than they've been for a period I can't recall the length, but suffice it to say, long. They are not totally 100%, however my feet are headed in the right direction. I soon will get my job back as a foot model.

Usually excellent. Only one time I came for an early 7:00a. m. Appointment and arrived a few minutes early. The door was locked. I think the staff came at seven or a few minutes later. I was just so shocked that I could make it a few minutes early (after driving almost thirty minutes) and the staff wasn’t a few minutes early to unlock the door. Only happened once.

The supporting team was patient, explained what's to come, uploaded my CD's with care, made sure I recieved it all back. All matters that make the stressful visit of someone who is experiencing chronic pain that much easier. Greatly appreciated.

I really like Dr Bob and his nurse/assistant. I scheduled surgery with him. Office parking in Sherman oaks is not the friendly. One woman in the waiting room was unhinged and yelling at people. I found that very upsetting.

Greatest doctor understanding and know what he’s talking about and doing much grateful. And after four doctors I trusted Dr.Baravarian for my surgery and his team specially the coordinator cheli helping before and after surgery thank you!

I was on state disability and Unum insurance. Several requests were made by Unum and myself to get the paper work to state disability dept and Unum in order to receive my benefits. You did respond to it and I got my benefits once. The request had to be requested again,, your office has yet to fulfill this request. When your office fails to do their part, I don't get paid. I am owed 2 months of back pay. PLEASE, I am respectfully requesting that you send state disability dept and Unum the paperwork needed. Each and every visit I had in your office, my co-pay was the first thing that was requested before I was seen, and each and every visit, I PAID IT! My Insurance paid you!
Thats why I had not responded to this survey. Hoping you'd come thru. As of now, I lack my benefits. If this is a concern that you'll sincerely take care of, please be so kind as to call me and inform me that you took care of this. (818)470-9625. This will help take the stress off of me.
If no call can be made asap, I will make an appointment to meet the billing dept or the office manager. I do need an appointment with Dr. Barabarian, perhaps he can help in this situation. Thanks for your time.
